Great idea. Do it. But I had it first, ages ago. So there.
You could include information like price for a credit(important) along with each entry. Make it worldwide. Other stuff I can't remember...Allow a facility for other users to verify each entry to back them up. You could have entries for actual arcades as well as the games they contain and allow comments for those arcades e.g. 'full of scumbags', 'broken joysticks' etc.

I was too tired to submit my interest when I first passed this thread. This was an idea I was preaching to ECM back when GameGo was getting started.
I'm very interested. And will help anyway I can.
I doubt you'd want to bother with a database of every game.
But please have a notation for whether the arcade has pinball or not :D

Chibi Nappa is right. I wondered if that was still there. I don't know anyone in StateCollege anymore, but last summer we were planning a road trip to go tubing and camping at a state park nearby, and that arcade was solidly on our agenda. This summer nothing's going to stop me.
Tempe Arizona had the arcade, but sadly I found out on the Go board that it closed in like 98 or 99.
There was a decent arcade on HighSt in Columbus Ohio, I heard that closed long ago too. :(
