Ok, so I'm a little late to the party, but what does the e-card reader do?
The E-Reader is used by several first-party GC and GBA games for importing data like new textures, new levels, items, powerups, etc. Also, there are some E-Reader cards that have first-gen NES games on them, and you can play them on a GBA.
Metal Slug Advance doesn't use the E-Reader (or cards for the device) at all. There are simply hidden items called E-Cards scattered throughout the game, giving MSA a 'collect-'em-all' element that's very common to portable games these days. SNK chose a pretty bad name for the cards, so now people are confused.
