Photoshop? Haven't you ever heard of Siamese twins?

This kind of thing happens a lot. Often times doctors can't seperate twins that are born this way because it will kill one or both of them due to one or more vital functions being shared by both twins.

Not every thing out of the ordinary you see on the net is a Photoshop job.
