i've got it installed and running in Vista currently. no patch. though i couldn't for the life of me get the hi-res patch to work (worked fine in XP).

bioware is currently working on dragon age, which is effectively baldur's gate 3 minus the d&d license (minus the crappy d&d ruleset, too).
