Yes, it's one of the more 'notable' Japanese customs that has even rubbed off on its share of homes in the states. I thought it was kind of awkward when I had to do it at first (BEFORE I even got to Japan), but now I understand and will continue the habit.
depends on the house ethics of the person I am staying at. For those that are very particular about dirt getting on their carpet and such, or those that express "mi casa es su casa" then yes. Otherwise, that's not too common among many here in North America...but ya, it's beginning to have its influence on Western families.

I pretty much always take off my shoes when I'm visiting someone else's home. They do the same in my home, too.

In my house yes always, if I go to someones house and they do it yes but if they don't there is no need to really. I definately don't want stuff stuck to the bottom of my shoes to end up on my carpet ;)
