You can contact Sony via email from their website. They respond pretty quickly
They need the Serial Number from your BBA (you have to remove the BBA to see the serial number because it is on the side you cannot see). You also need to know the date you bought the adapter.
Sony will research it and send out the game. In my case , the apologized and said it was mailed on Dec 26, so I am hoping to receive it soon.
