I haven't played the arcade enough to see the differences, but there's only a few like where some of the blocks are placed, etc etc. Otherwise it's pretty much the same as it's nes port.

Level layouts were ever so slightly different here and there. More bottomless pits, etc. 99.5% the same game.

Vs. SMB actually had some levels from the JPN SMB2 and when you finished the game, you could see all seven of the Mushroom Retainers surrouding Mario and Peach in the end. Otherwise, it's the same old SMB1.
A port of VS. SMB was actually released in Japan for the FDS titled All Night Nippon SMB (with a graphic hack that turned some of the characters into Japanese celebrities) and released as a premium.
Only Neozeedeater may know for sure...

Nintendo arcade games are some of my fondest gaming memories.
Playchoice 10 owned me for free in my childhood days.
