Lik-Sang is giving away ten copies of E3 Access 2004: The Future of Videogames 4-Disc Set, the same mega collection you can buy at GameSpot and other retailers. The answers to the first two you can find on the site of the DVD set's creator, NextGenVideos, and the third can be found with a smart Internet search (or by asking on these boards, I'm sure).

The collection is over fourteen hours long and covers all the sights and software on display at E3 2004.
