I was in a book store today, flipping through The Dark Knight design book. It has the full shooting script in it, and here's the description of Dent after the fall, pretty much word-for-word:
"Dent is lying on the ground with a broken neck. DEAD."
That's not to say they could change their minds and bring him back for a sequel, but I don't think they will. Dent was protrayed as a tragic character, and his dying at the end was very fitting.
So as far as TDK script is concerned, he is indeed dead.

I forget, have they even mentioned Arkham yet? It's been a while since Begins and I don't remember it being mentioned in this one.
Batman definitely mentions Arkham at some point, but I forget when or why.
*EDIT* It's the scene where Harvey Dent is threatening the Joker's henchman with the gun and Batman stops him. Bats tells Harvey that the guy is "a paranoid schizophrenic that the Joker picked up from Arkham."

Kevin Spacey would have made a good Riddler. Somebody earlier mentioned Phillip Seymour Hoffman for The Penguin, which would be a great transition as well.
