sometimes innocents get hit, accidentally or purposefully, and the worst part is there's no way to contest the ban so you're pretty well screwed unless you buy a new system (the console itself gets banned, not the gamertag)
edit: as far as that achievement goes, you could always start a new sp campaign and methodically go through each of the weapons since the counter carries... I triggered mine on a quickie co-op run after blowing up some wretches with the grenade launcher (also don't forget there's like 3 separate pistols, so make sure you use all them and have killed at least 1 dude with each)

well, that's not right. The inability to contest, that is.
That game last night was insanity, even if we were on the losing end of it. River is perfectly set up for a game of submission since it's one of the most symmetrical maps. Even weapons on both sides, power weapons in the middle, adjacent capture points, and a shitload of Gus mor-tar'ing the hell out of everyone out in the open.
