That's certainly part of it. The government forced companies' costs to go up, so the companies predictably and unavoidably are passing that along.

If by "forced companies costs to go up", you mean "forced companies to provide the services they said they would and people are paying for", then sure.

We've never been able to "keep our plan." Insurance companies change the plans every year. They change what they cover, they change the co-pays, deductibles, premiums, etc. Every year. It's how that insurance market works (auto, homeowners, etc. work similarly).
