Here's Hudson Soft's Takahashi Meijin in action on a rapid-fire tester. Damn. I can average 7 shots a second myself, or 8 if I'm lucky. He's going at 16 a second.

I heard a rapid-fire switch can be used to cheat your way out of this one with no problems (even if Ocelot says he'll know if you're using turbo). Not that I have done this myself.
Konami's International Track & Field on PS1 has a check for this though. If it seems you're using turbo due to an insane rate of fire, the game tells on you. "Using trick?" will come up above your power gauge. TM would probably get this.
Sega's Rambo arcade gun game has a rapid-fire QTE where you're running from a rockslide. If you don't fill the gauge in time by hitting the Start button as rapidly as possible, the rock hits you and one life is lost.
