The worst way to die.

Ripped to shreds, while being fully conscious.
Tell me that won't suck.

None of those last for three years.

I would say that starvation has to be the worst way to die considering the pain involved and the amount of time that it takes to actually kill you.

I think there's a lot of worst ways to die and many have been brought up already. How about a volcano erupts and being stuck on some rock that the lava will eventually reach?

You just have to stick your head in first, so you don't have to see/feel/smell your legs melting.

Again. How shitty does it feel to go without sleep for a day? Sleep deprivation gets exponentially worse over time. Imagine three years of it.

And these families know they have this gene, and know that every time they wake up, they could stay awake until they go crazy and die.

The worst way to die is to be eaten by a crocodile. They drag you under the water while clutching you in their jaws (extreme pressure too). So not only are you suffocated and drowned you're in a lot of pain while it happens.

All of this on camera would be the worst.

I don't see how being drowned and crushed is any worse than being drowned or being crushed. I think we have a finite capacity for pain...and having all those things happen at once would make each seem less intense.
