Well, I've only been playing for about five hours, so this isn't the most accurate viewpoint of the game as a whole, but so far, I think it's better than past GTAs as far as making the game's universe seem real due to the increased detail in the buildings and the environment as a whole, and the NPC actually behaving somewhat like real people.
Nice, but just window dressing.

However, hand-to-hand combat is worse than it's ever been - it's more limiting (so far anyway, I've only played for about five hours total, so I'm not sure if more attacks are available later), and it seems like there's a small delay from when you press the attack button to when you see the attack on-screen.

Here, driving is hurt by Rockstar trying to make the cars behave in a more "realistic" manner, which doesn't always jive with the missions (in particular, the chase mission), which have you driving in unrealistic ways.
Really, really important. I've gotten used to the driving, but the hand to hand isn't even as fun as in Bully. Fortunately I do most of my killing with guns or, even better, cars.
