Ico's cover was based on the art of Giorgio De Chirico, the co-founder of the Metaphysia school of art, a precursor to Surrealism. His work typically involved Italian plazas with odd lighting, enlongated shadows, and huge leaps of perspective. Some beautiful works.

Atlus has done some really great cover art. I think the only time they've ever screwed up was with Super Dodge Ball Advance (I wonder if they'll screwed RCR too).
The recent US Final Fantasy cover arts are better than the Japanese ones, due to the fact that the Japanese versions has none (unless you considering a blank background with a logo as art).

The cover art for the original Shenmue (U.S. version) is one of my faves. As is the JGR and JSRF cover art.
