This does indeed look fake. Look at the grass covered platform on the right, the ground below it looks all warped, like they screwed it from the original to make it look 3Dish, plus there is a line of bushes that should continue on underneath but are suddenly covered with that tan ground. Plus like NeoZ said, the character icon is the exact same thing from the box art.

People make fakes all the time -- it's so easy to fool people nowadays and fakers get props for fooling a lot of people. I'm in the this screenshot is fake camp. You can see everyone has a detailed shadow (that is pretty easy to create in Photoshop) except the little green guys from the first level in Gunstar heroes on the upper left. Also I think the background is taken from another 2d game--Astal or Rayman.
