It's an exploration of the darker sides of humanity. It takes guts to actually create those images and script those events - a majority of people enjoy blowing off their opponents head in an FPS, but if forced to actually create something of that sort, they balk. We did an experiment about it in a sociology course I was taking.
We were talking about violence in the media, and how we're a very voyeuristic society, then the prof asked everyone to draw/write the most disturbing thing they could think of for next class. A surprising amount of people refused to even do it.

Manhunt was a lot of fun when you included the voice recognition. Ok really just noise recognition, but it worked with the PS2 mic to allow a level of interaction that other games didn't provide. I remember watching my friend play the game with the headset on, he stalked his prey, got into position, then "YO BITCH!!" was heard, and the stooge dutifully came running over to see what the noise was, and then he was dead. And then the severed head was picked up and secured to the belt of the protagonist, until it turned blue about 20 minutes later, when we looked at it again.
Personally I think the game was a bit more graphic than it needed to be, but it wasn't rated teen or pg, it was rated M. I can't wait to see the new one, and I hope they include the voice recognition again.
Manhunt was largely violence-porn for fucked up adolescents, but with decent gameplay and ideas. Hopefully they take the forefront in the sequel.
