How about :

4v4 or 3v3 to eliminate host adv.
Default Weapons
Map to be agreed upon
10 sec BO
3 min rounds

Depending on how many teams we can get we can do the vote for captains and random out the rest/or pick.

The only way 4 vs 4 is capable of being fair is to give host to the newest player on both teams. Otherwise, I believe that either 2 vs 2, or 3 vs 3 with a moderator as neutral host is the most fair option. Depending on how many teams we should agree or poll the top 2-4 maps depending on how many brackets there are and assign appropriately.
