Health Insurance: I don't know exactly how not having insurance works as I have never been without. Some correct me if I'm wrong, no medical operation will let you die but I don't know payment specifics. As far as long term treatments, medical bills will stack up though.
Shoes: We take our shoes off at home. That's how my wife and I were raised. We make our friends take their shoes off but some of ours don't. Really depends on how you were raised. Not saying it has a positive or negative light to it, but I can see friends I knew from high school and college with homes of their own following in the same way as their parents' houses.
Broadband: In the northeast where I am, it's pretty much a given you have broadband. Haven't checked where my up and down is at as I have considerably slowed down with downloading stuff. As Yoshi said, the higher the population density, the more likely the area is fully broadbanded.
Religion: IMO, too much religion in our lives but I look at this way. I won't force this thought on others as long as those same people try and put more religion in my life. I deal with it.

The shoes thing is kind of weird. Usually if I go to someone else's house I keep my shoes on. In fact, that's always the case unless they are close friends or family. If it's a party or something the shoes always stay on. But if I'm just lounging around my house I usually hang out with no shoes on. Socks on if it's kind of cold.
People who force others to take their shoes off when they come in can eat dicks. I have a friend that does that and I really just make it a game to see how long I can keep my shoes on now.

Texas has free healthcare for the poor. What often happens to those with insurance (once they've reached their limits, which happens often in catastrophic cases) is they get on the Texas plan basically once they've sold off their assets and possibly gone into debt. Middle class people with good insurance can be wiped out entirely by diseases that are entirely covered so they can get treatment.
But then you're at the mercy of government health care, which sucks, has piss poor, underpaid patient advocates (people who talk to patients/families about their options within the system) and these days medical malpractice lawyers aren't too keen to take on cases ever since they capped damages (250k total, which sometimes doesn't even cover future treatment required b/c of the malpractice) in this state to supposedly get health care costs under control. Hasn't happened, but the medical insurers are posting a very healthy profit. I need to invest in them once I get some cash and watch for other states debating damage caps.
As for your religion point, except for legislation/political issues, religion doesn't play too heavily here even in Texas. People goto church, mosque whatever and keep it to themselves.
The broadband question I can answer, as I'm very much an internet geek.
Broadband penetration, speed, and availability depends on population density, as well as competition between providers. Unfortunately, in the large majority of the country, "competition" amounts to choosing between the local ILEC telco's broadband offerings (usually DSL), and the local cable company. There's some areas that have more than this, but this setup is typically what you'll find. The speeds vary drastically depending on where you're located. Verizon FIOS is pretty much the envy of everyone else, as they have a fiber solution that offers speeds drastically higher than everyone else. I believe their top package is 50/5 (downstream/upstream megabytes). Usually in areas that has FIOS, the competitor, usually the cable company offers comparible speed packages. In areas without FIOS, the speeds are typically much lower. In my area, the highest I can get is a 10/1 package from Roadrunner, and while that's plenty fast for most of my needs, that's a joke to people that have FIOS, and the typical internet speeds offered in some European and Asian countries. Bellsouth (now AT&T) highest speed is 6/768 (downstream megabytes, upstream megabits), mainly due to the limitations of antiquated copper, and the limitations of distance to DSL technology.
As for the religion question, that also seems to be based on area of the country. I live in the southeastern U.S, right smack in the middle of what's known as the "Bible Belt". Religion is omnipresent here, and there's about 6 churches within a one mile radius of my house. It isn't uncommon to see multiple churches on a single block. The Eternal Word Television Network (EWTN), which is the big worldwide Catholic television network is based here, and is about a mile and a half north south of my home. I've been by there, and the place is built like a fortress. Interestingly enough, Richard Dawkins is actually having a debate at the university here today, and this should be quite interesting.

Oh Broadband here is kinda meh. Didn't get it till 2003ish in houston, tx, and i get like 150k down at best and 50k up at best. Both subject to traffic shaping and throttling if they're torrents.
