I think it happens if your skills are on cooldown. I can make it happen consistently, but it's not like you need to change them up much. Still, I can't believe that made it past QA.
Maybe it will play out that way. Regardless, I think the acquisition of team members could have been woven into the central storyline with some craft, or at least broken up between major plot points (which would also provide an opportunity to tailor the story to people's choices).the "Empire" of the story
Also, while I find this much more satisfying an experience than Dragon Age, I miss having party members talk to each other...that was a neat touch. In ME2, they generally just make their remarks independently, but frame your other party member in a "thoughtful" way as if they're listening. I think there's specific dialogue for missions where a party member is required, but again, it's usually a comment about the situation and not directed at anyone (even the required party member).
